Tierra Rose Care Center is a family-owned and operated 76 bed Intermediate Care Facility located in a lovely setting in a quiet residential neighborhood in East Salem. We have a caring and supportive team of long term care professionals who work together to provide excellent care to our residents.
We have a new employment opportunity for a Director of Nurses with Long Term Care experience to lead clinical services within a long-term care facility (LTC) located in Salem, Oregon.
The DON / DNS will direct the nursing staff to provide the highest level of care while ensuring regulatory compliance.
Director of Nurses qualifications:
- This DNS position is for a Registered Nurse with 2+ years of extensive long-term care management experience and the leadership skills required to ensure the best care possible for all residents
- Extensive knowledge of state regulations with regard to long term care
- Stable employment history
- Experience with MDS 3.0 is preferred
- The DON / DNS will direct the nursing staff to provide the highest level of care while ensuring regulatory compliance
- The Director of Nursing must have a RN, Bachelor of Science in Nursing or Associate Degree in Nursing
- Working knowledge of MDS process, state and federal survey, and CMS is preferred.
- The Director of Nursing must have a current, unencumbered Registered Nurse license in the state of Oregon
- Excellent written and oral communication and motivational skills are essential for success
- Ability to creatively problem solve in both resident care and employee management situations
- Individual must be a team player, well organized, and flexible
- Ability to work on call and come in as and where needed
- Clinical oversight of nursing staff
- Coaching and mentoring the nursing team

The Charge Nurse is responsible for coordination of the resident's care according to the resident's Care Plan and as directed by the resident's RCM. The Charge Nurse is the direct supervisor for the CNA's, CMA's and other direct care staff for each shift. In the absence of the Administrator, Assistant Administrator, DNS and RCM's, the Charge Nurse is responsible for the overall supervision of the entire facility.
Duties and Responsibilities
The Charge Nurse is responsible for organizing and delegation of specific duties to assure that each resident's direct care needs are being met. This includes the following duties:
¨ Assuring that care is being provided according to each resident's Care Plan by being familiar with the resident's medical condition and assessments.
¨ Communication with direct care staff, RCM's and other Charge Nurses to assure that each resident's current status and conditions are addressed.
¨ Observation of resident care to assure resident safety and well-being.
¨ Receiving and implementing all physician's orders (medical and treatment) including: assuring all orders are written according to facility policy, following medication and treatment plans, documenting medication usages by residents, charting resident's responses to medication and treatment plans (including notification of physician when appropriate) and communicating orders to other departments to assure implementation.
¨ Assign, coordinate and supervise CNA duties.
¨ Assisting with and/or providing direct care to residents as needed for each shift.
¨ Assess residents for necessity of prn medication and monitor and document resident's response to the medication given.
¨ New Admissions: Documenting observations to assure that an appropriate Care Plan is initiated and implemented. Assure that admission orders are implemented and assist or complete an admission assessment as directed.
¨ “Alert Charting” on residents who are unstable.
¨ Observing resident responses to plan of care and charting of observations in the resident's clinical record.
¨ Reporting to the RCM, DNS or the resident's physician any change in the resident's condition and charting those changes in the resident's clinical record.
¨ Observation and documentation of target behaviors for residents on behavior monitoring.
¨ Documentation on the 24-Hour Report of acute problems or significant changes in resident's condition, medication/treatment changes (i.e., new orders received), behaviors, falls, etc.
¨ Maintaining a good working relationship with physicians to assure coordination of resident care.
¨ Recommending policy and procedure development and revisions as needed.
¨ Nursing Supplies: notify DNS of order requests, charging nursing supplies to appropriate resident accounts, assisting with inventory of nursing supplies and providing input on nursing supply budget.
¨ Participate in evaluation performances of CNA's and making recommendations at the end of probationary period and annual reviews for CNA's who have been supervised.
¨ Recommending training needs to the DNS for direct care staff.
¨ Communicate with consultants, facility co-workers and department heads as necessary to assure completion of duties.
¨ The Charge Nurse will complete any additional assignments, audits and duties as determined by the DNS, RCM or Administrator as necessary for resident care and to effectuate facility policies and procedures.
¨ Attendance at the following facility meetings is required: monthly Staff Meetings, nursing department meetings and additional seminars/inservicing as directed by the DNS.
¨ The Charge Nurse will attain and maintain a working knowledge of pertinent State and Federal regulations for Oregon long-term care facilities.
¨ The Charge Nurse will adhere to the Oregon State Board of Nursing - Scope of Practice for Nurses.
¨ The Charge Nurse shall have knowledge of facility policies and procedures and represent these guidelines to others at all times.
¨ Maintain standards of acceptable personal hygiene and adhere to the Nursing department dress code.
¨ Fragrance: Tierra Rose discourages the use of perfumes, colognes, after-shave or
scented lotions. If used, it should be sparingly so as not to affect resident and staff
who are sensitive to strong scents.
Tierra Rose personnel will at all times treat the residents of Tierra Rose with
respect and dignity and be an advocate for the resident's needs.
In representing Tierra Rose, all personnel must exhibit a professional,
caring attitude at all times. In keeping with Tierra Rose's philosophy of care,
the resident's needs are of primary concern. All issues related to the
residents and staff of Tierra Rose are to be treated confidentially.
Education: Oregon RN/LPN Nursing Degree.
Experience: Two years in long-term care preferred.
Skills Required: Must have the ability to read, write and understand
English. Highly motivated toward providing excellence in care to seniors. Belief in a positive, active and
professional attitude with staff members. Team oriented.
Must be in good health and able to carry out all
necessary functions of the position.
